Please read through the following guidelines prior to beginning the application as there have been a few changes since last year. Working on the Application
There are two required items you need to complete when you begin working on the application: your name and your email. This is the only way you will be able to return to the form at a later date.
You can print the application form at any time by going to the last page of the application and clicking on the "print" button.
To save the information you have entered into the form to work on at a later time, you will need to do the following things. (This is the same process you will follow when you are ready to submit your final form.)
At the bottom of the last page is a button to Preview your answers. You cannot Submit an application without first pressing the Previewbutton.
Once you preview your answers, click the Submit button. You will see a screen confirming that we have received your form. (If you do not receive a confirmation email, please check your SPAM/JUNK folder as it is likely to be there.) You will also receive an email from JotForm with the same links.
There will be two links on that confirmation screen and in the email: one is to Edityour form, and one is to get a PDFof your form.
Save the edit link so that you can return to your form to continue entering data.
Do this as many times as you need prior to submitting the final application. The application deadline is April 1.
Save the form to your computer with the name of your grant, e.g., Teaticketmathclub.doc.
Complete the form and then upload the completed form to page 5 of the application.

All proposals need the support of your Principal or Department Chair.
Please email the PDF of your grant application and a copy of your Budget form to your Principal or Department Chair, and to Dr. Sonia Tellier, Asst. Superintendent, email@example.com, when it includes a request for technology.
